Our Privacy Statement

Personal Information Protection Electronic Documents Act (PIPEDA) stipulates the following measures be adopted to ensure maximum client confidentiality.

1.      The information that Elixir Day Spa collects on its customers including name, address (home and office), age, birth date, medical notes, email, emergency contact details

2.      Accountability. The privacy officer of Elixir Day Spa is the owner and sole proprietor, Bilyana Dimitrovska. The responsibility of the privacy officer is to respond to enquiries, complaints, update information, understanding the legislation.

3.      Purpose. Information is gathered from clients for the following purposes:

·         to refer business to the individual

·         to generate an invoice

·         to be able to contact personally to give information for business use

·         to conduct training for clients and notify them of changes

1.      Consent. Consent is obtained from each client or vendor by giving them a form that explains the use of material and asks for their signature on the consent form. The consent statement explains that information will be used purely to advise clients of upcoming new services special offers and the like.

2.      Information collected will not be given to any organization outside of Elixir Day Spa.

3.      Limit Use, Disclosure & Retention. Information gathered is retained on the hard drive of one Elixir Day Spa computer owned by the Proprietor. Hard copies are retained in one filing cabinet in a home based office. The file cabinet is locked when not in use. Information will be destroyed by hard drive deletion should the computer become obsolete or at the bequest of the client. Information is gathered by the client filling in a form.

4.      Safeguards. The electronic database is password protected. The filing cabinet in the home office of Elixiir Day Spa. The Sony computer system has firewall and virus protection. Only one person has access to the information and no unauthorized parties can access information.

5.      Transparency. The privacy policy of Elixir Day Spa is published on the website at www.elixirdayspa.ca. It is also filed electronically on the computer and in hard copy in the filing system.  A printed copy of the privacy policy is available upon request.

6.      Access. Any client or vendor may request  a copy of the privacy policy by contacting  Biliyana Dimitrovska , 28 Jessica Drive, Barrie, ON L4N 5S5. Phone 705 770-9936. email: info@elixirdayspa.ca. The information will be provided within 5 working days.

7.      Recourse. To find resolution to a complaint, approach first the privacy officer of Elixir Day Spa. Should that prove unsatisfactory, individuals can contact the Privacy Commissioner of Canada, 112 Kent St., Ottawa, ON K1A 1H3.
